Provide culturally safe comprehensive primary health care to Aboriginal men in the Ntaria and Mpwelarre Health Service areas Conduct men's health checks and facilitate care in the early detection of chronic disease Operate as a Remote Area Nurse with an incorporated responsibility of case managing men An inherent requirement of the Ntaria position is travel to surrounding serviced communities including Wallace Rockhole and Utju Support chronic disease clients in the planning of care which includes self-management goals and as necessary coordinated team care arrangements Ensure all documentation, records and health information systems are maintained on the Communicare patient information system Is this role right for you?
