Our medical centres offer general practice, radiology, pharmacy, pathology, allied health and specialists – all under one roof.We are looking for an experienced registered nurse to provide support to our medical practitioners. The main focus of the role is to facilitate chronic disease management within the medical centre. You will also be required to support the treatment room nursing team by assisting with minor procedures, immunisations and other duties on an as-needed basis.This position will be located at our Gold Coast centre with plenty of opportunity for work across the week.Job tasks and responsibilitiesAssisting GPs with Chronic Disease Management.Support patients with a multi-disciplinary team approach to better self-manage their conditions.Organising case management for patients by liaising with internal and external health care professionals.Assisting GPs to prepare Care Plans, Health Assessments, Team Care Arrangements, Diabetic Reviews, Asthma Reviews.Assisting with minor procedures and immunisations.Skills and experienceAHPRA registered nurse with 3 years of experience in GP nursing.Experience with Chronic Disease Management.Knowledge of Medicare Benefits Schedule - Item Numbers for Care Plans and Health Assessments.Excellent written and oral communication skills.Driven, with the ability to identify and recruit patients and promote the importance of Chronic Disease care.If this position interests you, click apply now!
