About the role:
An experienced Registered Nurse with skills in chronic disease management activities to provide expert care and training to nurses in general practice, where all staff are supported in their learning and development. The successful candidate will play an integral role in achieving the successful completion of chronic disease management plans, reviews, and health assessments. You will provide high-quality clinical training and mentorship around chronic disease management in a general practice setting in a professional, friendly, caring, and supportive manner, according to best practice standards.
The successful candidate will have: Current AHPRA Registration – Registered Nurse (Division 1) General.Background working in a General Practice and/or primary health setting.Experience in Chronic Disease Management, Health Assessments, and an understanding of their utilization in the CVC Program.Experience with Best Practice (an advantage).Experience with MyMedicare.Capacity to travel to regional Victoria.Key responsibilities: Successfully complete GP Management Plans, Team Care Arrangements, and Health Assessments and applicable reviews in an agreed timeframe.Support/assist practices to identify appropriate patients as required.Support/assist practices to maintain a recall system for reviews.Assess patients and document results, identifying needs.Prepare GPMP/TCA and HA's.Understanding My Aged Care, Healthlink forms, and submitting MASS applications.Provide self-management information and other patient education.Understanding of preventive health and aged-based immunization requirements.Assist in referring patients to allied health services where appropriate.Ongoing assistance with reviews and re-assessment of patients.Understanding of MBS descriptors for CDM and HA item numbers. Ensure Medicare item numbers are claimed correctly.Model and mentor CDM processes to other nurses within Family Doctor.Other general duties as directed.The benefits:
