Join Junction Support Services as a Children's Case Manager! In this vital role, you will manage cases at multiple support levels, from initial assessment to enhanced case management, ensuring that each child receives tailored support. Your responsibilities will include conducting comprehensive assessments, developing and monitoring flexible case plans, and facilitating therapeutic group work. You will collaborate closely with referring agencies and external services to provide holistic support, advocate for the child's best interests, and enhance family well-being. This role involves working directly with children, assisting with their transition, and making referrals to specialized services when necessary. Your efforts will contribute to decreasing trauma impacts, improving family support, and fostering inter-service collaboration.
Key responsibilities include: Conducting comprehensive assessments and case planning. Providing enhanced case management and practical support to children. Monitoring and reviewing case plans, and developing exit plans. Assisting in therapeutic group work and individual counseling. Collaborating with internal teams and external agencies. Advocating for children's needs and ensuring effective service delivery. Selection Criteria A relevant tertiary qualification, such as a Bachelor of Social Work or Social Science, and relevant experience. Proven understanding and experience working with children and families, especially those experiencing homelessness, family violence, and trauma. Strong knowledge of the Case Management Framework and its application in a family-oriented approach. An understanding of children's developmental needs and the ability to prioritize appropriate support.
