Children's and Family Intake and Care Coordinator   Wellbeing and Primary Mental Health - Loddon Children's Wellbeing Local (LCHWL) Allied Health Processionals EA 2021 – 26 32 hours / fortnight Permanent Part Time   The Loddon Children's Health and Wellbeing Local (LCHWL) provides a range of health and wellbeing services to children up to 11 years of age who may be experiencing some challenges including, developmental, emotional, relational or behavioural concerns. Services are available to families across the shires of Campaspe, Central Goldfields, Loddon, Macedon Ranges, Mt Alexander and Greater Bendigo. The LCHWL is an innovative model of care that is co-designed with paediatric service providers and people with lived experience, to provide a system response that improves access to primary and tertiary care across the Loddon region. Services provided through the LCHWL include paediatric health, mental health support, parenting support, specialist assessments, allied health, family services and care coordination. The LCHWL has recently been established in response to the Royal Commission into Victoria's Mental Health System. The Child and Family Intake and Care Coordinator plays a key role in supporting accessible and coordinated services for children aged 0 to 11 years and their families across the Echuca Regional Health (ERH) catchment. This role is part of a larger care system within the Loddon Children's Wellbeing Local (LCHWL), a consortium of partner organizations working together to support system navigation and ensure timely access to primary and tertiary care for vulnerable children with complex needs. Key responsibilities of the Child and Family Intake Coordinator include: Intake and assessment: Handling initial inquiries from families and conducting assessments to determine the level and type of support needed. Strength-based and family-centered approach: Applying a holistic, child- and family-centered approach to care, focusing on the strengths and unique needs of each child and family. System navigation: Assisting families in navigating the care system, providing guidance and support to access appropriate services and resources. Coordination and collaboration: Working with other professionals and organizations within the LCHWL consortium to coordinate care and provide integrated support. No wrong door approach : Ensuring a seamless experience for children and families, regardless of the point of entry into the system, and facilitating access to necessary services without barriers. Follow-up and continuity of care : Following up with families to ensure continuity of care and ongoing support as needed.
