About the Role The purpose of the Child and Family Practitioner Mentoring and Personalised Support Service (MAPSS) is to engage and support young people referred to the MAPSS program to improve outcomes across areas of safety, health and wellbeing, social and emotional development, education, and learning. The incumbent works in partnership with the young person and their care team to develop and implement high-quality, individually tailored Care Plans, outlining goals which draw upon the young person's strengths and addressing identified needs.
The objective of the position is to support young people to achieve higher outcomes across ecological domains which correspond with the Child and Youth Wellbeing framework, using the Outcomes Star tool to capture the voice of the young person and their needs, and to assess and measure progress across these domains.
This is a Full Time 12 Month Max Term appointment based in Hobart. A Tool of Trade vehicle (including fuel card) will also be allocated to the successful candidate.
Key Responsibilities Develop and maintain functional and supportive relationships with young people and their care teams in order to promote positive outcomes.Utilise the Outcomes Star tool to complete initial assessment of strengths and needs, capturing the young person's voice and informing goals.Complete safety planning with the young person as necessary.Coordinate formal review meetings with relevant stakeholders.Support and advocate for the young person's safety and best interests at all times.Participate in regular supervision with the Senior Child and Family Practitioner and be open to peer supervision and feedback.Undertake regular reporting to referral agency outlining progress, barriers and case planning.Ensure the necessary Care Plan and risk assessments are developed and implemented.Keep case notes and other records as required by LWB and by law.Skills & Experience Recognised Social Work qualifications with eligibility for membership of the AASW and/or a Bachelor of Psychology or other relevant qualificationCurrent First Aid & CPR Certificate (or willingness to obtain)Demonstrated experience working with individuals, young people/families and communities from a strength-based, culturally appropriate perspective that are part of the out of home care sector.Demonstrated capacity to work autonomously in carrying out agreed Care Plans for multiple young people simultaneously.Demonstrated experience in working within a multi-disciplinary team.Demonstrated experience in managing competing priorities and to deliver high quality outcomes within agreed timeframes.Demonstrated understanding of impact of trauma and attachment on children and young people and complex behaviours.Ability to maintain positive relationships with individuals whilst being able to have honest and at times difficult conversations.Demonstrated ability to develop quality Care Plans that are consistent with appropriate policy and procedures.Demonstrated analytical skills and the ability to apply these skills in case management.Highly developed critical thinking and reflective practice skills and the ability to apply these skills in case management.Ability to effectively engage in peer supervision and receive robust feedback.Successful candidates will be required to clear probity checks including National Criminal History Record Check and hold Tasmanian Working With Vulnerable People Registration.
