Where you will be working: The Sustained Nurse Home Visiting (SNF) program is a coordinated and integrated initiative led by nurses, aimed at improving outcomes for vulnerable families and children through sustained home visits. As the successful candidate, you'll join a multi-disciplinary team of enthusiastic and dedicated clinicians delivering services from Raymond Terrace and Port Stephens to the Lower Mid North Coast. The role is currently based at the Raymond Terrace Community Health Centre.
The SNF model involves working in partnership with families to help them achieve their goals, with a focus on the wellbeing of the baby. This is done through regular, structured home visits and active follow-up with families. You'll also engage in weekly consultations and case discussions with the multidisciplinary team, and the role involves monthly travel to the Lower Mid North Coast.
About your new role: We are looking for a passionate and enthusiastic candidate, with a commitment to supporting vulnerable families. You'll be a reflective practitioner who works in a strengths-based and partnership-focused way, building therapeutic relationships with appropriate professional boundaries.
This role also involves completing the Advanced Nurse Practice (ANP) Course. You'll be supported by a Manager for Child and Family Services, the NUM/Coordinator for SNF, a Clinical Nurse Consultant (CNC) for Child and Family Health Services (CFHS), a Clinical Nurse Educator (CNE), allied health professionals, and your nursing colleagues. Orientation to the program will be provided at the start, and all SNF nurses must meet mandatory education requirements, including completing the ANP Course. The course includes a case study assessment and attendance at training days, and ongoing support will be provided by the ANP Course Coordinator
