Job Description About the role You will be responsible for supporting the delivery of the Medical Outreach Indigenous Chronic Disease Program (Aunty Jean's Program). You will be required to build community capacity through improved self-management for Aboriginal people with chronic health problems, appropriate partnership arrangements and culturally appropriate activities and self-management strategies.
Tasks include but are not limited to the following:
Collate clinic information and health information from program participants Assist participants with health measurements and sign in Promote and market the program to referral sources to encourage participation in the program Assist with administrative functions of the program, including but not limited to, developing information packs, following up referrals and appointments and coordinating venues and clinics when required Provide a cultural link to the community, program coordinator and exercise physiologist Organise transport for participants when required Support the development of care plans for referred clients to enable participation in clinics Participate in coordinating clinic activities Desired Skills and Experience Key Selection Criteria Previous experience in a similar position Holds or is working towards a minimum Certificate III in Aboriginal Primary Health Care, or a minimum Certificate III health qualification in the area of care in which the Aboriginal Health Worker works, and a willingness to participate in additional training as required Demonstrated knowledge and understanding of Aboriginal societies and cultures and the issues affecting Aboriginal people Demonstrated ability to communicate sensitively and effectively with Aboriginal and Torres Strait Islander people Ability to work flexible hours A valid Working with Children Check (WWCC) and a Criminal Record Check (CRC) issued within the last 12 months that includes working with vulnerable groups Salary Packaging Benefits Package up to $15,900 of your salary each FBT year, which can significantly reduce your taxable income and increase your take-home pay Package up to $2,650 of your meal and entertainment expenses annually per FBT year Salary Sacrifice Superannuation payments – boost your retirement savings Additional paid leave days to be taken during Office closure period at the end of the year Flare Benefit perks with exclusive discounts and offers on groceries, restaurants, entertainment and travel.
