An Eligibility List (E-List) or talent pool may be created from this recruitment episode to assist in filling future permanent, temporary, or casual full-time and part-time positions. A portion of this vacancy is maternity relief position and should the permanent incumbent require an early return from maternity leave, this contract of employment will cease with two weeks notice.
Maternity relief positions can be for a period of up to 12 months, with the possibility of continued employment for up to a further 12 months for maternity leave reduced hours, therefore there are no claims to permanency. Central Coast Local Health District (CCLHD) has openings for permanent & temporary full-time and part-time Registered Nurses to join the Hospital in the Home (HITH) team at Gosford and Wyong Hospitals and the Long Jetty Health Care facility.
In this vital role, you will serve as primary health care workers in the community, providing acute clinical care to patients in their homes or at HITH clinics.
You will work collaboratively with a multidisciplinary team to monitor health statuses, respond to episodic health needs, and promote health maintenance, rehabilitation, and disease prevention.

As a Registered Nurse in the HITH program, your primary responsibilities will include delivering acute clinical care to patients in their place of residence or at an HITH clinic, working in close collaboration with the multidisciplinary HITH team.
Close ties with both the community sector and the acute hospitals within Central Coast Local Health District will be required.
As the settings in which care is delivered, HITH nurses require skillful application and adaptation of evidence based practice on which nursing is based.

Your role will also involve educating patients and their families about their conditions, medications, and self-care strategies, empowering them to take an active role in their health journey.
Flexibility and adaptability are key, as you will need to assess and respond to a variety of clinical situations in diverse home settings.
You will actively promote health, recognise early signs of disease, and provide timely interventions to help patients maintain their independence and well-being.
Additionally, you will record data for service evaluation and contribute to continuous quality improvement efforts within the team.
About You You have current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse (Division 1) You have recent nursing experience in Acute Care Nursing or a demonstrated interest in Acute Care nursing. You have a current Drivers' Licence and willingness to travel within the Central Coast Local Health District as required. You have high level time management skills and an ability to effectively work both within a team and autonomously. Where you'll be working

You will be primarily based in patients' homes across the Central Coast, with additional responsibilities at Gosford and Wyong Hospitals and the Long Jetty Health Care facility.
This community-focused role offers a unique opportunity to develop strong relationships with patients and their families while providing care in a familiar and comfortable environment.
You will work closely with other health care providers, ensuring a coordinated approach to patient care that prioritises their health needs and promotes optimal outcomes.
