We have developed an opportunity for a Registered Nurse to work collaboratively with clinicians in our Community Nursing team at Woy Woy Peninsula sector. As primary health care workers in the community, you will be required to be involved in the continuous monitoring of the health status of communities as well as responding to episodic health needs.
Community Nurses, recognising the importance of social and ecological influences on the health needs of the individual, accept the responsibility to ensure that community nursing is relevant by developing responsive strategies to enable them to adapt to the changing health needs of the community.
About You You have a current registration as a Registered Nurse (Division 1) with AHPRA. You have recent nursing experience in Community Nursing or a demonstrated interest in Community Nursing. You have demonstrated ability to liaise with other health professionals to facilitate case management and care planning. You enjoy working as part of a team and have a focus on developing key relationships in a multi-disciplinary team.
