Are you passionate about making a meaningful difference in the lives of older adults experiencing mental health challenges? Join our team as a Mental Health Residential Aged Care Clinician and play a vital role in enhancing specialist mental health care within residential aged care facilities (RACFs).
As a Mental Health Residential Aged Care Clinician, you will provide expert mental health support to RACF residents, developing and implementing care management plans for those with high-risk or chronic conditions. You will work closely with a multidisciplinary team to assess, coordinate, and advise on complex mental health concerns, ensuring timely, safe transitions from inpatient care to RACFs. This role also focuses on preventing unnecessary hospital admissions and fostering strong partnerships with community stakeholders to deliver comprehensive, consumer-focused care.
This position is graded for the following:
Social Worker Level 4 Senior Psychologist Clinical Psychologist Occupational Therapist Level 4 Clinical Nurse Consultant Grade 2 About You Extensive Clinical Experience: You have a strong background in mental health assessment, treatment, and management, particularly with individuals aged 65 and over. Your expertise enables you to handle complex and high-risk mental health conditions. Leadership and Collaboration: You demonstrate clinical leadership and work well within a multidisciplinary team, supporting both colleagues and residents while fostering strong partnerships with external stakeholders. Consumer-Focused Approach: You are dedicated to a recovery-oriented, person-centred care model, ensuring that residents' needs and goals are prioritised in care planning and delivery. Effective Communication: You possess excellent communication skills, enabling you to clearly and respectfully engage with residents, families, and healthcare professionals, while also navigating clinical systems with ease.
