Cclhd Head To Health Kids Hub Aboriginal Care Navigator

Details of the offer

Employment Type: Temporary Part Time up to 30 June 2026 Position Classification: Health Clinician Level 3 Remuneration: Dependent on Qualifications Hours Per Week: 24 Requisition ID: REQ521092
Are you passionate about making a difference in the lives of Aboriginal and Torres Strait Islander children and young people? Join our team at Child and Adolescent Mental Health Services (CAMHS) and be a vital part of providing culturally sensitive care navigation.
The Head to Health Kids Hub provides comprehensive, multidisciplinary care for children under 12 who require secondary mental health and wellbeing support. These hubs aim to improve early intervention outcomes by identifying and addressing mental health challenges in children at an early stage. Offering a stepped approach to care, the hubs provide tailored support, treatment, and intervention based on evidence and validated assessment tools. The team, consisting of medical professionals, allied health staff, psychologists, and early childhood development experts, works collaboratively to deliver bio-psycho-socio-cultural assessments and interventions that complement and enhance existing services for children and their families.
About You

You are a proud Aboriginal person with degree qualifications in your area of specialty. You maintain all necessary professional registrations required to practice, including APHRA. You have relevant post-registration qualifications and at least 3 years of experience working in the clinical area of your specified post-graduate qualification. Alternatively, you possess equivalent qualifications or experience deemed appropriate by the public hospital or health organisation.
This is an Aboriginal and/or Torres Strait Islander Identified position, on the basis of a genuine occupational qualification for this role, as authorized under Section 14(d) of the Anti-Discrimination Act 1977, NSW. Applicants must demonstrate Aboriginality, in line with the three-part definition in addition to addressing the selection criteria.
You are a passionate advocate for inter and multidisciplinary care planning, serving as a crucial information connector within the Central Coast Local Health District (CCLHD). Your role bridges the gap between CCLHD staff, Chief Medical Officers (CMOs), and the private sector, fostering strong relationships and ensuring seamless communication.

About the Role

Family and Community Collaboration: Be the key contact for Aboriginal and Torres Strait Islander families at CAMHS. Work with families to create effective care plans, complete referrals, and liaise with schools and agencies for holistic care.
Enhancing Professional Skills: Improve the knowledge, skills, and confidence of professionals working with Aboriginal and Torres Strait Islander youth with neurodiversity and mental illness.
System Development: Develop systems, processes, and tools for consistent engagement, referral, and transition. Manage an allied health service map to streamline services.
Project Work: Lead projects to develop information tools for diverse audiences, consult on a collaborative information portal, and create individualised referral pathways for neurodiverse individuals, including children under 12.
