These teams include acute, community and rehabilitation teams in adult and older persons' services.
We currently have positions available at Elpida House, which provides medium term recovery-focused rehabilitation in a residential facility to support consumers in transitioning to community living.The Community Rehabilitation Worker (CRW) will support consumers in their rehabilitation and recovery goals through direct support for day-to-day tasks.
As a member of Elpida House, the CRW will work across various environments including the centre and the larger community to assist and support the development of residents' rehabilitation in partnership with their existing formal and informal networks.The CRW works under the general direction of clinical staff to provide a range of non-clinical and psychosocial rehabilitation support, in conjunction with the services provided by the professional/clinical on-site team.
Tasks will be practical in nature, supporting residents' accomplishments of their stated goals and facilitating improved functioning and community tenure, which may include helping to facilitate therapeutic groups.About YouWe are looking for a dedicated Community Rehabilitation Worker who has a passion for supporting individuals through their rehabilitation journey.
