Eunoia Lane boasts a very enthusiastic, passionate and committed team of professionals and we are searching for a pro-active and motivated Case Manager/Support Coordinator to join our team.
The position will be based in our Alice Springs office.
Regular travel to remote communities will be required.

About the position: The role of a Case Manager/Support Coordinator is to coordinate the supports for NDIS Clients and assist them in achieving their goals outlined in their NDIS funded plan.
This role is required to proactively identify, develop, coordinate and facilitate access to supports and services alongside their allocated NDIS Clients.
The position requires a highly organised person with excellent oral, written and interpersonal communication skills.
Required to work well both independently and cooperatively as a member of a team, the position requires a person who displays self-discipline with the ability to avoid distractions and diversions in order to manage their own time effectively.
Analytical and creative skills are necessary in order to analyse the individual needs of people with disabilities, with the ability to identify, measure and report on outcomes.
The candidate chosen to join Eunoia Lane as a Case Manager/Support Coordinator will have positive and contemporary values towards people with disabilities and must have the ability to interact, communicate and engage effectively with diverse cultures.
Key responsibilities: Work with clients, families and carers to identify and access opportunities for skills development, education and employment.Support and encourage clients to achieve their goals and provide support coordination services in line with their NDIS plans.Develop networks and collaborate with stakeholders to proactively identify, plan, implement and evaluate supports.Prepare and maintain up to date and accurate case management notes and reports using clear and concise language on client progress.Liaise with a wide range of stakeholders, community services, government agencies, customers and service providers.Attend and contribute to team meetings, discussions and planning.
