Case Manager – Sisters of St John of God Women's Program Outcare provides person centred care for people whose circumstances place them at risk of entry to the criminal justice system, or who are currently serving a sentence. The purpose of Outcare is to support people to regain direction and reintegrate into the community.
Our Sisters of St John of God program comprises a comprehensive suite of supports for women involved in the justice system and experiencing family domestic violence. The Case Manager position in our Sisters of St John of God program will work with clients and co-design support plans to remove barriers to successful engagement with community services.
The Role: We are seeking a Case Manager in The Sisters of St John of God Program. Successful candidates will be responsible for providing ongoing support and expertise for the planning and implementation of person-centered action plans that fulfill the reintegrating and re-connection of clients who have been, are returning from, or are at risk of further engagement with the justice system.
This will involve the following: Provide guidance and encouragement to clients to achieve agreed goals as detailed in their action plan. Encourage positive relationships and support networks through role modelling. Educate, encourage, and support clients to engage with internal and external support services. Support clients to identify short and long-term goals. Establish and maintain collaborative and supportive relationships with clients, service providers, other team members, and the local community through demonstrating respect, empathy, and the willingness to understand differences in culture and customs. Experience working successfully with clients experiencing systemic disadvantage as well as clients with a history of offending and re-offending. Knowledge and understanding of Aboriginal and Torres Strait Islander cultural customs, and protocols, and the diversity of issues affecting Aboriginal and Torres Strait Islander people in Western Australia, including rural and remote areas. Understanding of the issues related to involvement with drug and alcohol usage. Experience in establishing and maintaining positive and collaborative relationships with partner organisations and other stakeholders. The ability to communicate sensitively and effectively and work well with people. Experience supporting justice-involved individuals, or any other group experiencing systemic disadvantage. Detailed understanding of the challenges justice-involved women face. Ability to gain a successful prison clearance. Knowledge and understanding of the challenges of family and domestic violence. Other Employment Requirements: Western Australian Drivers license Police Clearance (within 3 months) – A criminal record does not make you ineligible to fill this position. Persons under current correctional supervision will not be considered for this role at this point in time.
