About the Role This role is a permanent full-time role in which you will be working as the Case Manager of our School Leaver Employment Support (SLES) program. In this role, you'll be at the heart of empowering NDIS participants as they transition from school into the exciting next chapters of their lives, whether that's starting new employment, pursuing further education, or joining Disability Employment Services (DES).
Delivered within a person-centred framework, the Case Manager will encourage participants to be active decision-makers in their lives and help strengthen and increase their community engagement.
The Case Manager will travel for work to different locations as needed and provide meaningful support to vulnerable individuals, making a positive impact on their future.
Key Responsibilities Manage an active SLES caseload, delivering services using a person-centred framework and established practice model. Develop one-page profiles to inform employment plans and coordinate worksite visits based on participants' career goals. Utilize community services and programs. Support work experience opportunities. Maintain accurate and timely records. Develop and maintain employer engagement and relationships to negotiate long-term employment opportunities for participants. Ensure compliance with health and safety requirements as per legislative and Barkuma policies. Participate in monthly supervision to support development and improvement. Support and advise the SLES Team Leader and deliver monthly reports. About You Successful candidates must bring with them:
Proven experience supporting individuals with cognitive disabilities. In-depth knowledge of the employment industry. Strong understanding of community services and available programs. Strong communication skills and ability to engage with people with diverse abilities, both verbally and in writing. Strong problem-solving skills. Demonstrated patience and persistence. Exceptional time management and organisational skills. Proficient in Microsoft Office Suite. Qualifications and Clearances: Certificate IV in Disability/Community Services or relevant area. NDIS Worker Orientation Module 'Quality, Safety and You' (or willingness to obtain). NDIS Quality and Safeguards Worker Orientation Module (or willingness to obtain). Current DHS Working with Children check (or willingness to obtain). Appropriate infection control certificate. Appropriate Child Safe Environments certificate. Appropriate First Aid + CPR certificate. Other Conditions: Work rights for Australia. Ability to work at any Barkuma location as directed. After hours/weekend work will be required. Current Australian Driver's Licence (restricted P2 licence and above) and comprehensive motor vehicle insurance. Don't miss your chance to be part of something amazing.
