Case Manager: PSP - Supported Independent LivingNewcastle, Newcastle, Maitland & Hunter NSWThis role is paid under the SCHADS Award Level 5The OpportunityThe Case Manager is responsible for all aspects of program delivery to ensure the safety and wellbeing of young people in the program.
They proactively engage with young people through regular home visits, professional and accountable casework, advocacy, and planning.

Applicants who are working towards this qualification will be considered if they have a minimum of three years' experience in a similar role.Demonstrated ability to use a holistic based approach in implementing and overseeing case management, case plans that incorporate appropriate models of intervention, create opportunities, empower and promote the wellbeing of the child/young people through engagement of families and carers.Strong interpersonal skills that provide the basis for effective negotiation, mediation, and conflict resolution practices and outcomes.A non-judgemental and empathetic approach to engaging and working with children and young people and their families.Demonstrated understanding of the complex needs of young people and families/careers facing leaving care.Proven ability to develop rapport with carers, child/young people, and their families/significant others in order to support informed decision-making and skills development for living within the community.Demonstrated commitment and ability to maintain confidentiality and accountability, including records of children and young people and their families, team, and organisational discussions.The ability to effectively develop and maintain positive working relationships and networks with other Government and Non-Government agencies/services.Demonstrated ability to participate positively in teamwork, training, and supervision processes.Demonstrated understanding of the relevant Government legislation applicable to the Supported Independent Living Program.Who we are?Formerly known as the Anglican Diocese of Newcastle, Newcastle Anglican is a faith-based organisation serving the people of the Hunter and the Central Coast.
