About the Opportunity As the Linkages Case Manager within our Carers Support team, you will manage a caseload that enables you to provide holistic and tailored case management to our consumers and their carers. You will build a village of supports around our consumers aged under 65 who have complex needs and require assistance to remain living in their home independently. You will have a positive impact on the consumer's health and wellbeing and enhance their ability to connect and participate in the community. You will develop plans and identify existing and new service opportunities to support consumers to meet their goals. About You You have proven experience in case management practices and have a deep understanding of the impacts of disability. You are energised by the impact you have on a consumer's health and wellbeing from finding those links to supports and services that address their needs and goals. You understand the services and guidelines provided under the Home and Community Care Program for Young People and a knowledge of service providers across mainstream and other community service sectors, including aged care, mental health and disability providers.
