The Case Manager is responsible for delivering person-centred case management to Alzheimer's Queensland's Home Care Package consumers based in Toowoomba.
The Case Manager will be required to complete initial intake assessments, develop and maintain care plans, perform holistic case management, monitor and review consumers under a framework of active partnerships with consumers and their family/carer.
You are required to apply the principles of wellness and reablement and facilitation skills to achieve quality consumer outcomes.
Responsibilities will include: To build strong relationships with consumers and their family/carer and are active partners in developing an effective support plan for the consumer Ensure packages are filled, waitlists are maintained, and budgets managed effectively Ensure consumer care and case management is performed in accordance with best practise standards, legislation and AQ philosophy, policies and procedures Where applicable and is within the scope of practice for your licensed profession, you will utilise your skills, knowledge and evidence-based approaches to deliver quality coordinated discipline-specific intervention to Home Care Package consumers ? We are looking for candidates with: Demonstrated Aged Care experience in a Case Manager, Care Coordinator or similar role Ability to work autonomously along with the capacity to operate and prioritise effectively Ability to communicate with a range of people including older people, staff, families and particularly in relation to care of consumers Relevant qualifications (Diploma level highly regarded) Excellent time management with established case load Extensive Home Care Package knowledge National Police Clearance Certificate Current First Aid & CPR Certificate Open Drivers Licence and car and williness to travel in the Toowoomba region and occasionally to Brisbane Remuneration This is a Full-Time position.
