Child Welfare, Youth & Family Services (Community Services & Development) Contract/Temp Glenview Community Services are a leading Tasmanian aged care and disability provider based in Hobart's northern suburbs.
We are part of our community and provide a diverse suite of services, including tailored home care, cottage respite and our state-of-the-art purpose-built dementia care village.
As a Case Manager, you will be at the forefront of our community care, providing high quality service and delivering a range of options that support the goals of our clients in their pursuit to maintain their best life.
You will work with team members to pursue and deliver excellence in supporting our community clients.
The successful candidate will ideally hold qualifications in home or community care and have knowledge of Home Care packages, CHSP and the NDIS.
The role is for a fixed term period of 12 months.
In addition, you will demonstrate: Strong financial, organisation, prioritisation and problem-solving skills.
Knowledge of the Home Care Package Program, CHSP and NDIS program requirements.
Highly developed communication skills with a diverse range of clients and stakeholders.
High level service quality and continuous improvement capabilities.
Change adaptability, flexibility and team-focused approach.
Essential criteria for this role include: Satisfactory National Police Check valid within 12 months.
Current Working with Vulnerable People Registration (NDIS Endorsed).
Eligibility to work in Australia.
