About the role

As the Case Manager, you will be responsible for delivering case management to our clients that is person-centred and enablement-focused. You will be involved in all aspects of care coordination, including administrative tasks and rostering of clients' services. You will develop positive working relationships within the local service centre and more broadly across the organisation to facilitate improved outcomes for our clients. We are currently seeking a Case Manager to join our supportive Home Care Team. What you bring

Tertiary qualifications in Community Services / Case Management or related disciplines. Experience in delivering quality, client-focused case management. Understanding of the various funding sources available in the aged care sector. High level of computer systems proficiency, including data entry. Experience developing care plans for clients with complex needs. Available to work Tuesday to Friday. A current valid driver's licence and insured motor vehicle.
