The Supported Temporary Accommodation (TA) provides a pathway to independence for people who are homeless or at risk of becoming homeless on the Northern Beaches of Sydney by supporting them to complete their Housing Application, explore accommodation options, and overcome other barriers they may face.
This role will assist clients residing in TA to access available housing options with the aim of rapid rehousing.
Your key responsibilities will be to:
Conduct comprehensive case coordination and assist clients in meeting the requirements of their Temporary Accommodation support plan.Undertake initial registrations and advocate to internal and external services to support clients transitioning.Create and update individualized case management files and undertake a range of case management duties.Develop strong relationships with key internal and external stakeholders and work respectfully to provide culturally appropriate support.Complete a range of other administrative duties and participate in relevant MA meetings and forums.Requirements for Success A relevant qualification in Community Services, Social Work, or a related field.Minimum 2 years of experience in a similar role.Driver's Licence.Excellent interpersonal skills with the ability to work cooperatively in a team environment and independently.Excellent internal and external stakeholder management with high-level communication skills.Before starting work with us, you will need to undertake a national police history check, qualifications, referee checks, and working with children checks.
