Working from a recovery-oriented model, the Mental Health Division provides an array of hospital-based, community and specialist services for infants, children, young people, adults and aged people throughout Victoria.About the roleAs a Case Manager, you will work collaboratively with the MSTS Consultant Psychiatrist and the team to provide the highest level of clinical care to the consumers. The service operates between 8.00 am and 9.30 pm Monday to Friday and 8.30 am to 5.00 pm on weekends and public holidays. Case managers work on shifts over the course of the week.A regular day will see you:Undertaking assessment and case management of NEMSTS consumers and contribute to the implementation, monitoring and evaluation of treatment objectivesDeliver high standard quality care for a clinical caseload, as well as consultation and liaison with family and organisationsProviding assertive mobile outreach for consumers in their local communityMedication supervision to the consumers in the communityTogether with the Consultant Psychiatrist and Registrar and the consumers planning clinical interventions that are appropriate for consumer and making sure implementation is done to the high standardsProviding knowledge and skills, based on professional background, as part of a multidisciplinary team, consult with other mental health staff on specialist and community psychiatry mattersAbout youYou will be someone who has excellent written and verbal communication skills, and the ability to work both autonomously and as a part of the team.
