Position Overview: As a Case Manager at TDN, you will play a pivotal role in coordinating services for a diverse range of clients. Your primary objective will be to support recipients of Home Care Package funding who aspire to maintain their independence and live comfortably in their own homes.

Key Responsibilities: Conduct integrated assessments to understand the unique needs of each client.Collaborate with a variety of healthcare professionals and service providers to coordinate an array of services.Establish and maintain strong relationships with key stakeholders involved in the client's services.Ensure that government funds are efficiently utilized within the bounds of legislation and guidelines.
