The program focuses on building pathways to social inclusion, education, and employment while fostering stronger family connections where appropriate.As a Case Manager, you will play a key role in delivering person-centred support to at-risk youth and their families.
You will provide guidance, make appropriate referrals to external services, and work collaboratively with young people to support them to make positive changes in their lives, creating opportunities to move away from homelessness and negative behaviours.You will work alongside clients to achieve goals with a holistic, person-centred, culturally aware approach relating to managing barriers relative to housing tenure and homelessness, education, or employment opportunities, so they can be ready to transition into long-term stable housing and gain independence.Your Key ResponsibilitiesRespond to referrals of individuals and or families from internal and external services (including FDV and AOD) and conduct initial assessments of suitability for support.Undertake formal face to face assessments of clients, including completing all necessary paperwork using the approved assessment tool.Work with individuals and families to develop a holistic case plan that is sensitive and responsive to their cultural strengths and identified needs and goals to increase tenancy outcomes, support networks, independence and wellbeing.Maintain professional boundaries of care practices while building the clients capacity.Provide intensive ongoing case management sessions (formal and informal) with clients and review progress against individual support plans.Link client to relevant support services which will assist in addressing issues associated with their homelessness including AOD, FDV, health, education, employment, recreation and social networks including referrals to supplementary services as needed.Undertake a range of case management duties to support the development of clients including referrals and support letters, interaction with other service providers, appointment setting and advocacy internally and externally.Requirements for SuccessWith a significant background in the housing, homeless and/or human services sectors, you will be confident in your case management and assessment processes.
Other requirements include:Degree in Community Services with at least one year's experience or Associate Diploma with relevant experience or less formal qualification with substantial years of relevant experienceExperience and/or knowledge of issues surrounding homelessness including alcohol and other drugs, family and domestic violence, unemployment and mental healthDemonstrated case management experience including assessment, case work, accurate and timely recording of case notes, advocacy, and referralComprehensive knowledge and ability to network and build relationships with housing, accommodation and community service providers and relevant government agenciesAbility to work with a range of client cohorts including individuals, youth, couples, families, women and children and have knowledge of family systems, with a focus on clients who are homeless and experiencing family and domestic violence, substance misuse and mental health issuesExcellent communication and interpersonal skills including verbal and written communication, with strong computer literacy in Microsoft Office suite, and other systems required to support the workReport writing and statistical collectionExcellent relationship-building skills and demonstrated ability to work as part of a teamWell-developed time management skills and the ability to manage a range of tasks at any given timeFull driver licence with no restrictionsBefore starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.Our CultureOur innovative and inclusive culture will ensure you feel valued at Mission Australia as you help Australians in need and develop in your career.We prioritise the safety and wellbeing of everyone, both within our team and the individuals we serve.We are committed to creating a diverse and inclusive working environment.
