In this role you will provide case management support to residents in the Open House Program.
The role works alongside clients/participants/residents to develop and implement case plans to achieve self-determined goals.This role participates in the professional operations and delivery of supported accommodation and psychosocial support for clients with low to mild mental health and episodes of intensive mental health needs.Your key responsibilitiesClients are supported and encouraged to achieve own goals within established case plans including taking clients to appointments and advocating for clients to other services providersUndertake intake and assessment with clients to develop a shared understanding of their situation and related problems and strengthsAccurate, up to date client records and data is maintained for all clients in accordance to TSA standards and procedures within TSA's identified electronic databaseEnsuring appropriate preventative action is taken in relation to potential high-risk situations, as per policy and procedureAdvocate on behalf of clients with various government and non-government agencies, both verbally and in writingEffective relationships are established with clients that encourage self-determination and participation This is a permanent full-time position, located in Darwin, NT.
Salary and conditions are in accordance with the SCHADS Level 4 Award.You will ideally haveDegree qualification preferred, in Social work, Mental Health or Welfare, Community Development and with related field work experience and demonstrated high level knowledge in the sector and/or skills attained through previous appointments, service and study/developmentOr a minimum Certificate IV in Community Services in Social work, Welfare, Community Development and demonstrated related high-level work experience of sector knowledge and skills attained through similar appointments, service, and study/developmentDemonstrated experience in engaging clients who encounter the Justice System and or Corrective Services and willingness to assist in the provision of support in the areas of homelessness, alcohol and other drug dependence, and mental health issuesA National Police Record check is required prior to commencementA NT Working with Children Ochre Card check is required prior to commencementA current NT driver's licenseBe able to provide proof of eligibility to work in AustraliaYour life at Salvos Maybe eligible to NFP salary packaging ($15,900 tax free) plus meals and entertainment benefits  Employee Assistance Program - Independent confidential counselling service Financial, retail and lifestyle discounts and benefits Discounted health and fitness programs through Fitness Passport  Up to 8 weeks leave per year through our purchase leave scheme Generous Parental Leave offering of 12 weeks  Up to 5 days paid leave per year to support a TSA program or activity An inclusive culture of dedicated, passionate and professional team members Positively supporting and impacting the lives of others through your career contribution Applications will close as soon as a suitable candidate is secured.
