About the Role Barnardos are looking for a full-time 12 month contract Case Manager for the Community Adolescent Program (CAP) based in Bruce, 2617 The Case Manager will be responsible for; Lead the care team in supporting the placement of children and young people, including developing, implementing, and reviewing care plans.
Provide case management and support functions in line with the Care Plan for young people, ensuring their needs are met.
Coordinate care teams of diverse service providers to sustain placements or support young people's transition to independence.
Develop and maintain relationships with key stakeholders in a young person's life, including mediating and negotiating with birth families to ensure consistent identity development.
Support the implementation of therapeutic care plans for young people, with a culturally sensitive approach, particularly for Aboriginal children and those from diverse cultural backgrounds.
Manage budgets for young people's needs, while committing to self-care, professional supervision, and maintaining work/life balance.
About You As a Case Manager, you will ideally have; A relevant tertiary qualification in Social Work, Psychology, Early Childhood Specialist and/or related behavioural sciences degree level with relevant experience.
Two years' previous experience in child welfare / youth work / community services / residential care Demonstrated knowledge and skills in working with families and promoting family centered strength-based practices.
Demonstrated case management experience including a complex/competing caseload.
Sound knowledge of child protection and issues in relation to children at risk particularly in regard to their safety, development, and connection to family, culture and community Ability to liaise with families, foster carers and professionals and to advocate for children/young people and carers.
High level of written communication skills, ability to prepare reports and correspondence to a high standard for a range of audiences.
Be available for after-hours emergencies and participate in the on-call and duty roster.

Salary: Base $73,804 to $85,131 per annum plus leave loading plus superannuation.
Packaging available to receive $15,860 of your salary tax free.
