Child Welfare, Youth & Family Services (Community Services & Development) Empowering Minds and Development is a Specialist Case Management team committed to implementing strength-based, trauma-informed, and culturally sensitive practices.
Our services focus on addressing barriers and reducing complexity in the NDIS space while assisting our clients in connecting with supports while building capacity and resilience in their communities.
In the role of a NDIS Case Manager, you will be required to establish strong rapport and working relationships with a range of stakeholders to implement a range of support services in line with NDIS plans.
Key responsibilities are: Engage in evidence-based best practice case formulation, assessment, reporting, and interventions for clients identified with complex needs.Manage an ongoing and long-term case load of clients, including achievement of KPIs.Undertake informed referrals to appropriate allied health professionals and/or service providers.Initiate and maintain productive relationships with clients, stakeholders, informal supports, and service providers who may present with differing priorities.Manage client's NDIS plan to ensure services are being delivered within their allocated budget.Develop and provide reports on outcomes and success indicators, within the required timeframes.Develop strategies which will address the identified needs of the client in consultation with their representative/s where appropriate, in accordance with funding guidelines.Support clients and their stakeholders to understand the supports in their plan, navigating NDIS processes, identifying and accessing mainstream community and funded supports.Manage accurate record keeping and the security and confidentiality of records and information in accordance with Information Privacy Standards.Qualifications & Experience: Knowledge and/or the ability to rapidly gain an understanding of NDIS, disability, and community services sector.Exceptional customer service, negotiation, and rapport building.Strong interpersonal skills and high levels of resilience.Excellent writing, comprehension, and verbal skills.Ability to interpret information guides, processes, policy documents as well as clinical reports.Demonstrated skills and knowledge in the area of planning, implementing, reviewing, and evaluating effective service plans.Have creativity, a positive attitude, eagerness to learn, and willingness to adapt.Blue Card & NDIS Worker Screening Check.Degree in Social Work, Psychology, Counselling, Criminology, or related field.Previous experience working with a number of stakeholders.Application Requirements: Do you have customer service experience?Which of the following statements best describes your right to work in Australia?How many years' experience do you have as a case manager?Do you have a current NDIS Worker Screening Check?Do you have a current Working With Children (WWC) Check?Do you have a current Police Check (National Police Certificate) for employment?Do you have a current Australian driver's licence? #J-18808-Ljbffr
