As part of our dynamic team, you'll play a pivotal role in helping participants achieve personal goals, secure stable accommodation, and rebuild relationships, all while reducing the likelihood of reoffending. This exciting opportunity involves providing intensive case management, developing culturally appropriate Reintegration Care Plans, and collaborating with internal and external stakeholders to deliver the best outcomes for our participants.You'll lead the charge in conducting risk assessments, offering therapeutic interventions, and convening case conferences, all while building strong relationships with participants and other team members. Your work will also include promoting PPRSS to referral agencies, engaging with partnership organizations, and ensuring thorough documentation in line with Mission Australia's policies. With opportunities for ongoing professional development through training and supervision, this role offers a chance to contribute meaningfully to the community while advancing your career.Your key responsibilities will be to:Develop, implement, and regularly review culturally appropriate Reintegration Care Plans tailored for each participant.Collaborate with team members to complete necessary risk assessments.Provide participants with brief therapeutic interventions and facilitate referrals to appropriate services.Lead case conferences with key stakeholders to minimize service duplication and ensure optimal outcomes.Deliver outreach services to participants, assisting them in transitioning to independence or other support services.Foster strong internal relationships with participants and staff, contributing to the service's effectiveness and improved outcomes.Build and maintain strong relationships with key external stakeholders.Participate in program marketing activities, engaging both existing and new referral agencies.Collaborate with partnership organizations to support PPRSS planning and participation.Conduct promotional activities involving participants and the broader community.Perform a variety of duties to support participants, including documentation, external referrals, support letters, interactions with other service providers, appointment coordination, and advocacy.Requirements for SuccessExtensive experience in the justice or community services sector, working with individuals who have complex needs.Demonstrated expertise in a person-centred, recovery-focused, and strength-based approach to participant care, with a deep understanding of prison release processes and community and family reintegration.Highly developed verbal and written communication skills.Experience working both autonomously and as part of a multidisciplinary team, with the ability to navigate diverse inter-organizational relationships.Proven ability to manage workload and time effectively, including the ability to set and achieve priorities in a high-demand work environment.Advanced keyboard and computer skills, with competence in Client Information Management Systems.Experience working with Aboriginal and Torres Strait Islander people in a community service setting, along with a strong knowledge of Aboriginal and Torres Strait Islander culture.Valid NT driver's license.Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.Our cultureOur innovative and inclusive culture will ensure you feel valued at Mission Australia as you help Australians in need and develop in your career.
