Position Overview: We are seeking a dedicated Case Manager to join our Home Care team at Chris Barnard Health.
In this role, you will support our home care clients by leveraging your strong clinical and relationship-building skills.
Collaborating with your colleagues and suppliers, you will oversee the health and wellbeing of our clients, empowering them to remain in their homes longer.
As a proactive clinician, you will address their evolving needs with attention and care.
Strong time management, prioritization, and organizational skills will be vital, alongside the ability to identify when a client requires further assessment.
Who We're Looking For: We are seeking a Registered Nurse, Enrolled Nurse, or Allied Health Professional with experience in the home and community care sector.
A genuine passion for enhancing the health, independence, and lifestyle of the aging population in Melbourne is essential.
This opportunity is ideal for individuals who thrive on building strong relationships with their clients and leverage those connections to deliver the best possible care by effectively coordinating with other multidisciplinary stakeholders within the team.
Desirable Qualifications:Tertiary qualifications in Social Work, Aged Care, Disability Services, Community Services Coordination, Nursing, or an Allied Health discipline, with current AHPRA registration.Ambulance/Paramedic qualifications or experience.Experience in case or care management within Home Care Packages or Community Home Support Programs.What You'll Do:Work closely with clients, their families, and our support network to create personalized care plans that promote dignity, choice, and independence.Build lasting partnerships with clients and stakeholders, including clinical experts and allied health professionals, fostering a spirit of collaboration and teamwork.Conduct thorough assessments and develop tailored care plans focused on enhancing wellness and reablement, ensuring our clients receive the care they need to flourish.Support a diverse caseload, from Level 1 to 4 package recipients, addressing a variety of care needs.
