About the Program Family Support Service Greater Darwin is a partnership between CAAPS and Anglicare NT and provides support and assistance to families to keep their children safe, reunify children in care with their birth family and support pregnant mothers to grow up healthy and safe children. Families are supported to build relationships, strengthen parenting skills, maintain a safe home environment for their children and increase their participation within community.
About the Opportunity This position has dual management arrangements between Anglicare NT and CAAPS and is located at CAAPS.
As the Case Manager, you will play a crucial role in delivering strength-based, trauma-informed, and culturally affirming practical in-home support to families primarily referred by Territory Families Housing and Community (TFHC). Your focus will be on child-centered, family-focused casework, aiming to create nurturing environments, enhance parental capacity, and uphold family unity. This Case Management role requires flexibility and periodic after-hours work.
You will: Provide intensive in-home support to high-risk families, focusing on building their strengths and resilience. Implement trauma-informed practices to address the complex needs of families and children. Collaborate closely with families to develop and implement individualised support plans. Advocate for families to access essential services and community resources. Ensure families connect with their community, culture, and country. Foster positive outcomes through respectful and participatory practices. Build and maintain collaborative relationships with service providers and community stakeholders. Note:  This is an Aboriginal and/or Torres Strait Islander designated role. Therefore, only Aboriginal and/or Torres Strait Islander peoples are eligible to apply.
