Proven experience supporting Aboriginal and/or CALD clients is essential, candidates with Aboriginal and/or CALD heritage will be prioritised.Both Full and Part Time positions available.You will have significant active outreach experience, be a confident and capable case manager, able to undertake assessments, develop and manage against support plans, have excellent case noting (ISBAR), and make best use of your networks to proactively access services and supports.A copy of the job and person specification is available on our website www.communitytransitions.com.au. The role is Level 5 within the SCHADS Award which is currently $97,377.80 per annum (pro rata).
