The Role: Within the Tasmanian Health Services framework:
Provide case management services to older persons identified as medically stable and ready for discharge from the Acute Care Sector who would benefit from time-limited, goal-oriented, therapy-focused restorative care within the Southern Region. Comply with the Department of Health and Ageing Transition Care Program Guidelines, legislation, and reporting requirements. Duties: Link, refer, and coordinate services through care plans. Monitor service delivery. Collaborate with a multidisciplinary team to enable a person to meet their goals. Case closure and evaluation. The Team: The Tasmanian Health Service provides the Transition Care Program in Southern Tasmania, which seeks to optimise the functioning and independence of older Tasmanian people after a hospital stay. Transition Care is goal-oriented, time-limited, and therapy-focused. The Transition Care Program team focuses on delivering excellence in healthcare and partnering with a range of service providers to achieve improved outcomes for our consumers.
Details of Appointment: Permanent full-time, day worker position, working 76 hours per fortnight, commencing as soon as possible. *Notwithstanding hours to be negotiated with the successful applicant.
Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full-time, part-time, and casual vacancies.
Salary: $79,529 - $91,517 per annum. Our employer 11.5% superannuation contribution is on top of this amount.
Salary range is in accordance with Public Sector Unions Wages Agreement 2022. You'll have access to salary packaging and a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible.
