The Transition Care Program (TCP) team focuses on delivering excellence in healthcare and partnering with a range of service providers to achieve improved outcomes for our consumers. The Role: Within Tasmanian Health Services framework:
Provide case management services to older persons identified as medically stable and ready for discharge from the Acute Care Sector.Comply with the Department of Health and Ageing Transition Care Program Guidelines, Legislation, and Reporting requirements.Duties: Link, refer, and coordinate services through care plansCollaborate with a multidisciplinary team to enable a person to meet their goalsCase closure and evaluationThe Team: The Tasmanian Health Service provides the Transition Care Program in Southern Tasmania, seeking to optimise the functioning and independence of older Tasmanian people after a hospital stay. Transition Care is goal-oriented, time-limited, and therapy-focused.
Details of appointment: Permanent full-time, day worker position, working 76 hours per fortnight, commencing as soon as possible. Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full-time, part-time, and casual vacancies.
Salary: $79,529 - $91,517 per annum. Our Employer 11.5% superannuation contribution is on top of this amount.
Salary range is in accordance with Public Sector Unions Wages Agreement 2022.You'll have access to salary packaging and a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible.
