Description A unique position has become available as we expand our Home Care Package portfolio in Portland & Hamilton, Victoria. We currently have two full-time ongoing positions available as well as a fixed-term contract till 10/11/2025.

The Position : The Case Manager is a full-time position based in Portland or Hamilton. The successful candidate will be responsible for the initial provision and case management of Home Care Packages for clients. The position is pivotal to directing client care covering appropriate intake and assessment, care planning, community engagement and documentation, monitoring and review, fund management, and advocacy as required.
A dynamic role in our Home Care team, working in close partnership with our Clinical Team. You will form part of a collaborative, agile team with a refreshing approach and culture, as we strive to support our clients to live life as they choose.
Requirements Essential : Experience in case management / the provision of aged care and / or home care, typically 3 years+Sound knowledge of Home Care and / or Aged Care industry including exposure to the Aged Care Quality Code of Conduct, Aged Care Standards & accreditation processStrong communication skills, demonstrated ability to build rapport with internal and external stakeholdersClient-centric approach and willingness to advocate on behalf of clientsProven ability to work independently, make sound judgements, and prioritize tasks in a fast-paced environmentExperience in budgeting and funds managementDemonstrated understanding of risk management protocols, covering hazard identification and incident managementProficient computer and information management skillsAbility to problem-solve and resolve conflictCurrent National Police Certificate (or ability to acquire)Current driver's licenseDesirable : Certificate 4 in Home & Community Care or Aged CareBachelor's Degree (Desirable - Registered Nurse)Exposure to workforce planning & scheduling principlesTo be considered for employment with Respect, each candidate must satisfy the legislative requirements of Aged Care Worker Screening to ensure suitability to work in the aged care industry.
