For many Australians living with a disability, reduced mobility and/or cognition, in-home care and support allows them to live comfortably in their own home and actively participate within their communities.

About the RoleWe have an exciting opportunity to join Colbrow Care's case management team as a Case Manager servicing the Greater Melbourne Region.
The office is situated in our brand new purpose-built facility in Hawthorn.
The role consists of managing and growing our base of NDIS, HCP and Private Clients.
The Case Management role generally works autonomously with a large focus on client interaction, visitation, and management.
The successful candidate will be expected to attend the head office in Hawthorn on a required basis to liaise with other members of the Care Partner / Case Management team.

Candidates with prior Registered Nurse experience are highly regarded.
Responsibilities IncludeProvide thorough 1:1 assessments of clients within their homes and formulate tailored Care Plans to enable field staff to deliver safe, effective care and nursing services.
Registered Nurses will be available to oversee and provide guidance on clinical matters.Client intake and onboarding.Manage client queries and concerns in collaboration with the respective Manager.Contribute towards a culture of high-quality care through demonstration of leadership including but not limited to identification and management of hazards in the home.Communicate ideas on complex and sensitive issues clearly in writing, selecting language, tone, and format to suit target recipients.Identify areas for procedural improvement and provide coaching and/or training to internal staff and/or stakeholders as needed.Build rapport with a wide range of interest groups, with differing agendas and values, developing and maintaining good working relationships for the longer term.Assist in business development opportunities.To be successful in this role, you will need to demonstrate the following:Registered Nurse with AHPRA registration preferred.Experience in care partnering or case management in the Health, Aged Care, Disability or Community Services Sectors.Tertiary qualifications in Social Work, Aged Care, Disability Services, Community Services Coordination, Frontline Management or equivalent (Desirable).Proficient with internal databases, Microsoft Office, and government report funding portals.Current Victorian Driver's Licence (which must be maintained throughout the course of employment).Current NDIS Worker Screening Check.Current Working with Children's Check.Current First Aid Certificate (HLTAID011) and CPR (HLTAID009).Valid working rights in Australia.Mandatory competencies (as required by Colbrow Care).Why Join UsSupportive, friendly and collaborative team.EAP (Employee Assistance Program).Access to a fully maintained company vehicle.Employee referral program.Competitive remuneration along with career progression.Ongoing support and training from department leads and senior management.Company celebrations such as birthday recognition, Christmas parties, Melbourne cup event, charitable causes, and others.Team bonding activities.To ApplyPlease send your CV and a cover letter addressing the selection criteria.
