Housing Plus is a leading NFP Community Services Provider with a 40-year history, providing housing, domestic violence, homelessness, and other community services in NSW.
With offices in Orange, Bathurst, Mudgee, Dubbo, and Parkes, and services reaching the Central, Western, and Hunter regions of NSW, our purpose is to achieve healthy, engaged, and vibrant communities in which individuals enjoy opportunities to thrive and build a promising future for their families.We are currently looking for a Case Coordinator to join our Initial Transitional Service (ITS) team based out of Broken Hill.
This position can be 3 days/week or 5 days/week depending on the preference of the successful candidate.
We also offer flexibility in working from home.The ITS program is aimed at supporting individuals with complex needs (such as mental health, acquired brain injury, alcohol, and other drugs) upon initial exit from custody.
The program works with individuals for a period of up to 12 weeks during the critical transitional period to support reintegration into the local community through linking with identified services, building capacity in the individual, and developing a network of supports.Working in close partnership with a Corrective Services NSW Case Manager and engaging with other service providers and partners, the ITS Case Coordinator will be responsible for the day-to-day implementation of a case plan to provide collaborative service delivery to support parolees.To be successful, you will ideally have relevant experience in case management across a range of community service fields such as disability, mental health, and/or alcohol and other drug sectors, with a demonstrated understanding of best practice approaches in dealing with disadvantaged members of the community and a proven ability to professionally and sensitively deliver client services.
The ability to work with and understand issues affecting Aboriginal clients and clients from a CALD background is essential.Relevant qualifications in community services or similar will be highly regarded.You will need a current Driver's Licence and willingness to obtain a Police Check and Working with Children Check.We can also offer you benefits such as learning and development opportunities, a supportive team culture, health and well-being programs, reward and recognition, and salary packaging.This is a contract position to 30th June 2025 (contract renewed since 2018).
