Overview As a Case Coordinator for Home Care Packages (HCP), you will play a pivotal role in delivering a high-quality Community Care Program. This involves the identification of needs, assessment, implementation, coordination, maintenance, and evaluation of home and community-based programs. Your efforts will ensure that our clients—those who wish to remain in their own homes with some support—receive the best possible care and services.
Key Responsibilities: Program Development: Design and develop community-based programs tailored to the needs of ageing and disabled individuals.Assessment & Coordination: Assess client needs, coordinate services, and implement care plans in collaboration with a multi-disciplinary team.Evaluation & Review: Regularly review and evaluate the effectiveness of programs and services to ensure they meet the required standards and deliver optimal outcomes.Client Support: Provide comprehensive support and guidance to clients, helping them navigate services and resources effectively.The Region The Eyre and Far North Local Health Network (EFNLHN) covers almost 440,000 square kilometres of spectacular South Australia, supporting about 40,000 people living around Eyre Peninsula and the Far North of the state.
