Carer Support Liaison Officer Location: Goulburn Mulwaree, NSW; Shoalhaven, Illawarra Shoalhaven, NSW
Job Type: Full time
Salary: $75,545 to $79,385 per annum + Super + Salary Packaging (dependent on skills and experience)

Your Opportunity The Integrated Carer Support Service links carers with supports and services to help manage daily challenges, address stress, maintain wellbeing, and plan for the future. The Carer Support Liaison Officer plays an important role within the Carer Gateway program, providing support by liaising with service providers, Carer Support Planners, internal teams, and carers to arrange planned and emergency supports.
Key Responsibilities: Coordinate and liaise with Carer Support Planners and Service providers to arrange in-home or facility-based supports for carers in a timely manner. Coordinate care for carers, including planned respite, emergency respite, personal care, domestic assistance & lawn maintenance. Provide relevant referral information to service providers working with carers, within consent and privacy guidelines. Network with external stakeholders and service providers to maintain positive relationships. Source appropriate brokered supports for carers as requested by carer support planners/program manager. Engage with the community care sector and the public to raise the profile of the program and carers at regional events and activities. Maintain an up-to-date register of providers with a Service Provider Agreement with Carer Gateway/Mission Australia. Complete all necessary administrative tasks associated with care coordination and program activities, including accurate database entry. Requirements for Success: Qualifications and training in Community Services/Health/Welfare/Social Services and/or Education or a minimum of three years of relevant industry experience. Demonstrated knowledge of the community care sector, including challenges faced by carers of people with a disability, chronic illness, mental health condition, and the aged. High-level computer skills including client management systems and the Microsoft Office suite. Ability to provide superior customer service with strong interpersonal and communication skills. Demonstrated ability to manage and prioritise workload in a constantly changing environment. Current NSW Driver's License. Before starting work, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
