About the Role Carers SA is seeking 2 dedicated Carer Coach and Connectors to join our team in the Limestone Coast region.
These roles focus on supporting unpaid Carers by delivering strength-based, person-centered coaching and facilitating connections to resources and communities.
You will play a key part in assessing needs, setting goals, and empowering Carers to achieve positive outcomes while raising awareness about available services.
Key Responsibilities: Conduct assessments to understand Carers' unique needs and strengthsDevelop and implement coaching and goal-setting plans with CarersFacilitate Carer peer group sessions to promote social connection and resilienceAdvocate for Carers, providing support in accessing services, including emergency respite, counselling, and community engagement opportunitiesMaintain accurate documentation, including case notes and action plans, using Carers SA's systemsActively engage in reflective practice and professional developmentPromote the Carer Gateway and build awareness of support services within the communityTo excel in this role, you will ideally have: Recognised qualifications incoaching, social work, mental health (at minimum Certificate IV level), health, education or allied healthExperience in strength-based practices, goal setting, implementation and support maintenance of action plans, and community engagementStrong interpersonal and communication skills, with the ability to build trust and adapt to diverse groupsAn understanding of trauma-informed care and mental health support systemsThe ability to work independently and as part of a regional, mobile and collaborative teamExperience facilitating groups or similarExperience working in the human services sectorAn understanding of the issues facing CarersExcellent oral communication skills and the ability to establish rapport with a diverse range of peopleProficiency in information, communication and technology practicesYou will also need to have a current:
National Police ClearanceWorking with Children CheckFull current SA Drivers LicenseChild Safe Environments (Through Their Eyes) training, or be willing to undertake this trainingWhy Join Carers SA?
