About the Role:
The Career Medical Officer will be part of the Residential Aged Care In-Reach (RAC-I) service with a specialised aged care focus. The CMO will provide clinical care in collaboration with the Nurse Practitioner and registered nurses team, to maximise patient-centred care and quality of life by providing a proactive, integrated, specialised urgent care to Residential Aged Care Facility (RACF) residents within their usual accommodation. Service provision will be face to face or via virtual care.
This RAC-I will sit alongside and complement other Hunter New England Local Health Care services that already support our older population.
By joining our team, you will play a vital role in providing high-quality, patient-centred care as part of the RAC-In Reach multidisciplinary team. You'll deliver specialised urgent care services to residents in Residential Aged Care Facilities (RACFs) within their familiar surroundings. This innovative model emphasises proactive, integrated care to reduce hospital transfers, ensuring residents receive timely and effective nursing and multidisciplinary support. You'll also collaborate closely with RACFs and their usual GPs to ensure continuity of care and enhance the quality of life for residents.
Minimum Requirements: Qualified medical practitioner registered in Australia. At least three years post-graduate experience and appropriate training and experience to undertake: (1) Care of patients with multiple medical problems in a community setting. (2) Management of heart failure consistent with National Heart Foundation of Australia and Cardiac Society of Australia and New Zealand guidelines. (3) Management of chronic obstructive pulmonary disease (COPD) consistent with "COPD-X" guidelines. (4) Home visits within the Greater Newcastle Sector in an HNE vehicle as required. High level skills and involvement in clinical care, supervision, mentoring and teaching. Able to exercise a moderate degree of professional independence. Experience in Community General Practice would be an advantage but is not essential.
