Job Summary Carealot is seeking an experienced and motivated Community Manager to join the Great Southern Home Care team. Reporting directly to the organisation's Clinical Manager, this position provides leadership and management to the effective delivery of all aspects of safe and high-quality home care services to our Home Care Package and Department of Veterans Affairs Clients, their families and employees.
Key Responsibilities/Requirements Excellent written, verbal, and interpersonal communication including the ability to develop positive relationships with key stakeholders - clients, family members, and other representatives, Aged Care Assessment Teams (ACAT's), doctors, social workers, and other health care professionals. Customer-focused work ethic and a strong commitment to Carealot's Mission and values. Demonstrated ability to manage a Home Care Team to meet daily timelines and client requirements to efficient and effective standards and to achieve goals set out in Carealot's strategic plan. Demonstrated ability to successfully assess, plan, implement and evaluate client care and provide all aspects of clinical support and supervision to the Great Southern Home Care Team. Demonstrated knowledge and understanding of continuous quality improvement, quality standards, and the accreditation processes. Demonstrated ability to effectively manage change in a care-related environment in line with operational and strategic goals. Strong administration and computer skills. Minimum 5 years' experience supervising or managing a team. Ability to foster excellent teamwork through exceptional leadership, communication, and interpersonal skills and ability to build trust, credibility, and rapport across all facets of the organisation. Sound working knowledge of safety and health legislation, the Aged Care Act (1997), Home Care Package funding, and Department of Veterans Affairs funding (desirable). Excellent understanding of Aged Care Quality standards and audits processes including the Serious Incident Response Scheme. Current Registration with AHPRA as a Division 1 Registered Nurse. Current national police clearance no older than 3 months old or willingness to obtain. C class driver's license. Work vehicle provided during working hours.
