Based at one of our Home Care Offices in the area, in this dynamic role you'll manage a caseload of Home Care Package (HCP) and Commonwealth Home Support Program (CHSP) clients, driving towards exceptional care outcomes. Your expertise will ensure that each client's needs are met with top-quality service and support. You'll be the key player in coordinating care strategies and using your in-depth knowledge and skills to make a real difference in our clients' lives. Key responsibilities will include:Assessing the needs and preferences of each client in a caseloadDeveloping personalised care plans in collaboration with clients and their familiesCoordinating the delivery of services such as personal care, meal preparation, cleaning, and transportationMonitoring the implementation of care plans and adjusting as neededEnsure CHSP services are provided in accordance with CHSP guidelinesAdvocating for clients' rights and preferencesConducting regular reviews and reassessments as needed to ensure that clients' needs are being met effectivelyProviding information and support to clients and their families regarding available services and resourcesLiaising with healthcare professionals, service providers, and community organisations to ensure coordinated careWhat will you bring to the role?Experience in a similar role within Aged Care or Disability Services SectorQualifications in Nursing or Allied Health (desired not essential)Demonstrated knowledge and experience in delivering services under Home Care Package ProgramDemonstrated knowledge and experience in delivering services under the Commonwealth Home Support Program (CHSP)Demonstrated knowledge and experience of working in accordance with Aged Care Standards and legislationAbility to manage time efficiently and prioritise responsibilitiesExcellent communication skills.
