About this role As Care Navigator, you will provide an integrated and seamless care pathway utilising existing and future services to facilitate timely discharge from hospital and to avoid unplanned hospital admissions.
The role will work closely with hospital services as well as community services to support vulnerable older patients.
The Care Navigator is responsible for coordination and management of Care Navigation Services and overseeing the Frailty Rapid Access Clinic (FRAC) and Rehabilitation and Aged Care Intervention and Liaison Service (RAILS).
You will also be required to provide an advanced clinical service in triage and screening referrals to the Older Adult programs of FRAC, RAILS, Day Therapy Unit (DTU) and Geriatric Residential Outreach (GRO) at SCGOPHCG.
As we are part of a multidisciplinary care team, applicants may be required to work flexible hours as per business and patient needs.
There may be a requirement for weekend shifts.
This position is based at Osborne Park Hospital with the potential to work at alternate locations as the service develops.
What we are looking for We are seeking a highly motivated individual, qualified in either Occupational Therapy, Physiotherapy or Social Work with demonstrated relevant experience in geriatric multidisciplinary care to join our Allied Health team at Osborne Park Hospital.

Who we are Osborne Park Hospital (OPH) provides specialist health care services for the community in the north metropolitan area and is located in Stirling, 14 kilometres from the Perth central business district.
The hospital provides a comprehensive range of specialist health care services for the north metropolitan suburbs.
The services at OPH are provided by three main specialties consisting of Rehabilitation and Aged Care Services and Younger Adult Rehabilitation, Surgical Services and Women's & Newborn Services.
Each of these specialties has a dedicated Allied Health Services Team.
