The Opportunity We have a permanent full-time position available in the North of Tasmania as a Care Manager / Nurse Manager.
The role of the Care Manager is to support the Facility Manager in assisting Care Staff to provide a range of clinical care and support services in collaboration with care recipients, their families, and other members of the health care team. Our Nurses assist care recipients to experience a greater level of independence and ensure their health and wellness are supported.
As a Care Manager, you will provide supervision and monitor Registered Nurses, Enrolled Nurses, and Care Workers, and exercise initiative in regard to clinical care matters to consumers in accordance with:
The Aged Care Quality Standards The Code of Conduct for Aged Care The Charter of Residents Rights Organisational Policies and Procedures The successful applicant will need to demonstrate sound clinical skills, managerial skills, be consumer-focused, have a solid understanding of aged care accreditation processes and an appreciation of current and future aged care funding models.
Duties & Responsibilities Coordinate and oversee Clinical Care within the service, ensuring to maintain compliance with relevant regulation and legislation. Coordinate and oversee in consultation with the Facility Manager, the Continuous Improvement, Occupational Health and Safety, and Training/Orientation programs, to ensure the overall delivery of care meets the Residential Aged Care Standards. Effectively manage incidents, hazards, and near misses, and ensure impact is minimised with appropriate actions taken to improve systems, work practices and the working environment. Provide guidance and assistance to subordinate staff, acting as a role model for the provision of care. Participate in the resident admission/entry, review, and evaluation assessment processes. Coordinate liaison between the residents, their families, unit staff and other health care providers to ensure comprehensive input into resident assessments and care. Coordinate resident assessments, care plans, reviews and documentation ensuring that the care for each care recipient is systematically planned, implemented and evaluated and that care delivered, supports independence, and is consistent with contemporary best practice. Consultation with medical practitioners, allied health, and other health professionals to ensure positive outcomes for care recipients. Provide recommendations and coordinate referrals, following clinical assessments, to specialised health professionals where necessary. Establish and maintain effective and collaborative working relationships with other staff, residents, their families, and specialised health professionals for complex clinical conditions. Ensure significant issues and changes in residents' health is professionally conveyed to the Facility Manager in a timely manner. Coordinate daily staff allocation, activities, and routines to ensure a consistent approach to care and other services in accordance with residents' care plans. Maintain accurate and efficient documentation in a manner consistent with professional guidelines, Southern Cross Care (Tasmania) Inc. policies, the Aged Care Standards and relevant legislation. Oversee care documentation processes, including assisting and training subordinate staff to accurately document care delivered. Monitor and identify AN-ACC reviews, including ensuring that documentation is proficient, to ensure consumer care funding is optimised. Coordinate and oversee the management and ordering of clinical equipment and supplies, to ensure positive outcomes for residents.
