Management (Community Services & Development) Full time Ozcare is looking for a new Care Manager Lead to support the Care Manager team to develop and grow their Home Care Package business within their branch. Your Role Reporting to the Branch Manager, you'll be responsible for providing leadership to a dedicated Care Manager team in the planning, managing, and leading client care and support across a range of programs. You'll also play a key role in coordinating the involvement of Ozcare's multi-disciplinary teams and other services to support effective care. You will play a pivotal role in ensuring consistency in governance and compliance while driving service growth.
As a Care Manager Lead you will: Manage and coordinate care services to ensure the highest standards of careDevelop and implement care plans tailored to individual client needsLiaise with clients, families, and healthcare professionals to ensure comprehensive careFoster strong relationships with clients, stakeholders, and team members, while assisting clients in meeting their unique needs and goalsEnsure compliance with all relevant legislation and organizational policiesMonitor and evaluate care services to continuously improve quality and efficiencyMonitor the budget of Home Care Packages and ensure clients understand their funding and feesWork closely with the Branch Manager and Care Managers to ensure service growthProvide supervision and support to the Care Manager teamHow You'll Benefit You'll become part of the Ozcare family, and in addition to the training and support that you would expect, you will also receive:
