Calvary Home Care - Care Manager Make a difference at one of Australia's leading health care providers and within our local community Generous salary packaging benefits, and discounts Ongoing professional development and advanced career opportunities Permanent Part Time - 4 days a week About the role The Care Manager is responsible for delivering case management that is person-centred and enablement-focused, in accordance with Calvary Home Care policies and procedures, program guidelines, the National standards of practice for Care Management, the National Code of Ethics, and the Calvary Home Care Management Model.
Key Responsibilities Time management and working within expected time frames Understanding the Aged Care Standards Liaising with stakeholders and internal networks Monitor and respond to changes in clients' requirements with a person-centred approach Utilization of innovative and problem-solving skills What you bring Demonstrated experience in the provision of Case Management services Demonstrated experience in the aged/community/disability sector, particularly working with clients with complex needs Driver's license Highly developed proficiency using computers, including the Microsoft Office suite, with the ability to learn and adapt to new databases Understanding of the financial principles of individualised client funding and experience in monitoring and reviewing individualised budgets An ability to work in a team and negotiate with internal and external stakeholders to ensure accountability for service delivery NDIS clearance (or willingness to obtain one) Police clearance (within the last 6 months or willingness to obtain one) For further information, please contact Recruitment at ****** Why work for Calvary?
