Holistic Horizons is expanding into the Logan area as well as into the Aged Care community space. We are looking for a strong passionate leader to guide the company into this space through accreditation and launch. Position Overview The Care Manager is a key leadership position within the Services division and is primarily responsible for providing clinical leadership and direction to all Aged Care staff across all service deliveries, ensuring that day-to-day clinical work is carried out effectively to meet the needs of our clients. This role contributes to the development of evidence-based and innovative clinical services and will promote the delivery of care that is of the highest quality. Key Responsibilities Actively engage in clinical and caseload reviews. Manage referrals and demand for clinical services. Provide direct clinical services where required. Support Holistic Horizons community services financial outcomes. Lead a multi-disciplinary team effectively. Maintain close and effective working relationships with key internal and external stakeholders.
Work Environment This role will suit someone who prefers a hybrid model of mostly working from home during the launch of Aged Care and Disability in Logan, and someone who lives in the Logan Area. A Car will be supplied as part of this role. Full Salary Packaging as a Public Benevolent Institution available. Minimum Requirements To be successful in your application you will need to evidence: Registration with AHPRA as a Registered Nurse. NDIS Worker Screening Check. Working with Children's (WWC) check. Current Driver's Licence. At least 3 years experience in Managing Home Care Packages. Experience in a management position.
