Summary: As a Care Manager in Residential Aged Care, you will lead all clinical care aspects for our residents, fostering a warm, safe, and compassionate environment.
You'll supervise the clinical care team, offering mentorship and support throughout their professional journeys.
Your role will also include coordinating care plans and ensuring compliance with care standards, all with the aim of delivering the highest quality of care possible.
Key Information: Oversee the daily operations of the care facility, ensuring adherence to all regulatory requirements and company standards.
Create and implement personalised care plans that address the unique needs of residents, collaborating with healthcare professionals and families.
Lead and mentor the care staff, offering guidance and opportunities for professional growth and development.
Efficiently manage budgets, resources, and supplies, maintaining a focus on cost-effectiveness and quality care.
Conduct regular assessments and audits to ensure that facility services consistently meet the highest standards.
Address and resolve concerns or complaints from residents, families, or staff promptly and with empathy.
Requirements: Registered Nurse with a current AHPRA registration.
Proven experience as a Care Manager (or similar position).
Strong knowledge and understanding of the Aged Care accreditation standards.
Comprehensive knowledge and experience with ANNAC.
Strong leadership, communication and interpersonal skills.
An outstanding proficiency in care planning, goal setting and evaluation documentation.
Capability to effectively lead and manage a team.
