About The Role
We are looking for a passionate AHPRA registered professional (Nurse, Occupational Therapist or Social Worker) to join our fast-growing team in South Australia. In this role, you'll manage relationships and oversee service compliance for a portfolio of clients who have chosen The CareSide to fully manage their Home Care Packages.
Your Responsibilities

Complete client assessments and care plans
Mitigate client challenges and facilitate a smooth home care experience
Supervise care delivery
Review client progress notes and ensure corrective actions are implemented
Meet with prospective clients and explain the value proposition of joining The CareSide
Report and manage incidents and hazards
Develop relationships with clients and their families and proactively seek feedback to ensure we're meeting their service expectations
Collaborate with the scheduling team to ensure staff and client expectations are met
Chair regular team meetings with staff to support them in their day-to-day duties
Conduct performance reviews with your team
